Julie Rennison is the Acting Area Manager for Home Link, a suite of nursing and allied health programs providing bed hospital substitution and sub acute services in the Perth Metropolitan area.
With a nursing career spanning over 30 years Julie has had the privilege of working with patients, carers and families across the continuum of care.
Julie’s previous 10 years’ experience in the UK as a practice nurse in a busy inner city practice serving a diverse population ignited her passion for chronic condition management. She was engaged by the NHS Modernisation agency in the early 2000 to streamline resources, reduce waste and build capacity in primary care for chronic condition management.
Julie has seen first-hand the challenges patients face dealing with multiple chronic conditions, medication management and life priorities. She recognised, valued and implemented opportunities to work in partnership with the other community health care professionals, particularly with community pharmacists. After moving to Perth in 2004 Julie worked as a Clinical nurse specialist in the North Metropolitan Chronic Disease management team where she embedded Home Medicines Reviews as a core component of medication safety.
In her role as Acting Area Manager for Home Link , Julie’s creative solution focussed attitude encouraged the development of an early supported post discharge CoNeCT pharmacy service. With the introduction of ABF in WA Julie has been instrumental in facilitating the CoNeCT pharmacy business model to support this service, improving care across the continuum to provide a connected, person-centred health journey.
